Illinois Masonic Children’s Assistance Program (IMCAP) is dedicated to providing a brighter future for the children and young adults of Illinois Masonic families. Our focus remains on our Masonry roots of promoting community and human welfare, and helping our brethren and their children. Each program through IMCAP was specifically designed to encompass the “whole child” and the unique needs of children and young adults. IMCAP does not just assist with putting clothes on a child’s back and food on the table, but we also offer opportunities for the child to grow in all areas.

How to Apply:

A parent or legal guardian must complete an application for the child or children. Applicants 18 or older may complete the application themselves.

  • Download and complete the PDF version of the application below.

  • Sign the Compliance Agreement.

  • The child/young adult applicant(s) must have the identified need detailed in the application.

  • Family must show financial need.

  • Send completed application by

    • Email IMCAP@ilmason.org

    • Mail: IMCAP, 2866 Via Verde, Springfield, IL 62703

    • Fax 217-529-0242.

Before completing the document, save the form (PDF format) to a location on your computer or phone (for example: Desktop or Documents) You must save the file to your computer or phone first and then fill it out. Do not complete the form in your web browser or your data will NOT be saved.

  • Click on IMCAP ONLINE APPLICATION button below. This will open the form in your web browser.

  • Right click on form, select “Save As”.

  • Save the form to a location on your computer or phone.

  • Open the saved form.

  • After you have filled out the form, save this completed form to your computer or phone.

  • Send the COMPLETED form to imcap@ilmason.org.

 

What to Expect After Applying:

After submitting an application to IMCAP, your application will be reviewed and an IMCAP case worker will reach out to schedule a phone conference to learn more about your child/children’s or young adult’s needs.

The IMCAP case worker may provide referrals to relevant services that are related to needs. Additional financial information may be required (i.e. checking account statement, pay stub, unemployment benefits letter, etc.).

After gathering information, the IMCAP case worker will prepare a Request Report that will be presented to the Board of managers to determine an assistance plan during the monthly Board meeting. The decision of the Board will be communicated to you via phone or email.

Confidentiality:

IMCAP understands that it can be hard to ask for assistance. We ensure confidentiality with every family we help by issuing each family an unique case ID for request reports. The only individuals that will know you or your child/children’s names and identifying information will be IMCAP case workers who are assigned to your family. The Request Reports that are presented to the Board of Managers will only include your unique case ID, brief overview of needs, and summary of financial situation to show financial need.
